well, this one doesn't bug me but when Frodo is being kept in Cirith ungol and he is topless, you can see his boxers, though I quite liked that mistake.

I know this was done just for dramatics but Denethor running out on fire and jumping off the edge of the cliff.(it has a name I can't think of) Getting there from the House of the Stewards was quite a distance and someone doused in oil and burning would never have got halfway at best. It's petty I know.
